A tummy tuck is a surgical procedure that removes excess fat and skin from the abdomen, leaving you with a more toned and attractive figure. In addition to the removal of excess fat and skin, your surgeon will also tighten your abdominal muscles. This will give you a flatter stomach with smooth, contoured skin that is free of stretch marks or loose skin.

Abdominoplasty, known more commonly as a “tummy tuck,” is a surgical procedure to remove excess skin and fat from the middle and lower abdomen and to tighten the muscles of the abdominal wall. While a tummy tuck is very effective at producing a flat tummy, it can result in a significant scar, possibly extending from hip to hip.

A complete tummy tuck usually takes two to three hours, depending on the extent of work required by each tummy tuck patient. A mini-tummy tuck may take an hour or two.

Most commonly, the surgeon will make a long incision from hipbone to hipbone, just above the pubic area. A second incision is made to free the navel from surrounding tissue. With mini-tummy tuck, the incision is much shorter and the navel may not be moved.

Next, the plastic surgeon separates the skin from the abdominal wall all the way up to your ribs and lifts a large skin flap to reveal the vertical muscles in your abdomen. The surgeon then tightens these muscles by pulling them closer together and stitching them into their new position. This provides a firmer abdominal wall and narrows the waistline.

In a mini-tummy tuck, the skin is separated only between the incision line and the navel. This skin flap is stretched down, the excess is removed, and the flap is stitched back into place.

What Is Involved In A Tummy Tuck

The tummy tuck, also called abdominoplasty surgery, is among the most effective cosmetic procedures that help achieve a thinner and more defined waistline. This surgery entails removing excess fat deposits and the resultant hanging skin around the abdomen. The surgeon will also tighten the abdomen’s connective tissues and muscles, resulting in a more toned appearance.

The length of the procedure varies depending on the amount of excess skin and fat that needs to be removed. Your surgeon will also take into account how much muscle tightening they need to perform in order to achieve their desired results. The average tummy tuck takes between two and four hours to complete. During this time, you will need to remain still while your surgeon works on your abdomen. Some surgeons use general anesthesia while others use local anesthesia with sedation medication provided by an anesthesiologist or nurse anesthetist before surgery begins so that they can monitor your vital signs throughout the procedure while remaining conscious enough to answer questions if necessary during recovery period afterwards when full healing has taken place then returns home within 24 hours after surgery completes.

The tummy tuck procedure can be broken down into four main steps:

  1. Making an incision around the belly button and removing excess fat, skin and tissue
  2. Tightening the muscles of the abdomen by removing fat and excess skin
  3. Re-positioning internal organs such as the liver, spleen and intestines back to their original positions (this step may be done using a special device called an endoscope)

Steps of a Tummy Tuck Procedure



Step 1 – Anesthesia

Medications are administered for your comfort during the surgical procedures. The choices include:



  • Intravenous sedation

  • General anesthesia

Step 2 – The Incision

A full tummy tuck requires a horizontally-oriented incision in the area between the pubic hairline and belly button. The process includes the following:



  • The shape and length of the incision will be determined by the amount of excess skin.

  • Once the abdominal skin is lifted, the underlying weakened abdominal muscles are repaired.

  • A second incision around the navel may be necessary to remove excess skin in the upper abdomen.

  • The upper abdominal skin is pulled down like a window shade.

  • The excess skin is trimmed, and the remaining skin is sutured together.

  • A new opening for the belly button is created.

  • The belly button is popped through to the surface and sutured into position.



Step 3 – Closing the Incisions

Sutures, skin adhesives, tapes, or clips close the skin incisions.

THE TUMMY TUCK PROCEDURE

The tummy tuck procedure your cosmetic surgeon will use varies depending on your desired goals. The following are some of the critical steps involved in any tummy tuck

  • Applying general anesthesia or intravenous sedation
  • Making an incision on the targeted abdominal area
  • Lifting the skin and repairing the weak abdominal muscles
  • Trimming the excess skin and finally suturing it together

TYPES OF TUMMY TUCK SURGERIES

A cosmetic surgeon may recommend one of the following tummy tuck surgery procedures to help give you a firmer, flatter, abdominal contour with minimal scarring:

  1. Mini Tummy Tuck: If you’re mainly concerned about stretch marks and loose skin below your belly button, then the mini tummy tuck surgery is the ideal option. This procedure involves making one horizontal incision below your belly button, from which your surgeon then effectively tightens the loose abdominal muscles. Surplus skin is also removed to create a flat and firm abdominal wall.
    Mini tummy tuck surgeries usually take a relatively shorter time and will often have a faster recovery time, along with minimal scarring. Expect a thin scar, usually a couple of inches, along with a firmer and flatter lower abdomen.
  2. Full or Classic Tummy Tuck: This tummy tuck addresses both the upper and lower abdomen areas and will typically yield great results in removing excess fat. Your cosmetic surgeon will use either a U-shaped or horizontal incision below the belly button or the navel. Excess fat and skin are then eliminated to achieve a slim abdomen that looks natural.
    The full or classic tummy tuck will typically leave a longer scar due to the extensive incisions required. However, the scar will hide below a swimsuit bottom. This type of tummy tuck benefits includes a considerably smoother, slimmer abdomen with fewer stretch marks and more comfort wearing skirts and pants.
  3. Extended Tummy Tuck: The extended abdominoplasty is the standard tummy tuck that includes recontouring the flanks, hips, or even lateral thigh. It is quite useful for those who have experienced significant weight loss and are now dealing with excessively saggy skin on their abdomen. In most cases, the surgeon will perform liposuction around the flanks during the tummy tuck.

WHAT TYPE OF AFTERCARE IS REQUIRED?

Your doctor will discuss your required tummy tuck aftercare with you, including the following:

  • You may have surgical dressing and small tubes to drain excess blood or fluid along with your injection site, which may be left in place for a few days.
  • You may be prescribed pain medication, an antibiotic, and blood-thinning medication.
  • You’ll need to wear an abdominal binder to support your stomach for about six weeks.
  • You’ll also need to be careful when moving around, so you don’t strain your incision line and reopen the wound.
  • You’ll need to come to follow-up appointments with your doctor.

No-Drain Tummy Tuck

In older tummy tuck surgery techniques, the space between the skin and muscle was left open and unattached, providing a space where fluid could collect. As fluid collected, causing a seroma, the healing process would slow down, so surgeons would prevent this by inserting small tubes, or drains, to remove excess fluid an prevent swelling.

But living with a drain for several days, even weeks, after abdominoplasty is inconvenient and uncomfortable.

That’s why at the Plastic Surgery Clinic we have a developed a no-drain tummy tuck surgery. This new technique, called Progressive Tension Abdominoplasty, is a form of drainless tummy tuck that seals the space between the skin and muscle where fluid tends to accumulate. Using a special kind of internal stitch called the Quill suture, the skin and muscle are locked in place with the barbs of the suture.

Your consultation will involve a physical examination, including a careful medical history, and a discussion about your goals with surgery. Dr. Lista, Dr. Ahmad or Dr. Austin will spend as much time as you need to make sure both you and your surgeon know exactly what you want. It may not always be possible to meet all your expectations with surgery, so it is crucial that you understand what is a realistic outcome.

If, for example, your fat deposits are limited to the area below the navel, you may require a mini-tummy tuck, which is a less complex procedure that can often be performed on an outpatient basis. Or your surgeon may recommend having a tummy tuck in conjunction with liposuction to remove fat deposits from the hips for a better overall body contour. Or liposuction alone may create the best result. Your surgeon will work with you to recommend the procedure that will help you come closest to achieving your desired body contour.

Your surgeon will also instruct you on how to prepare for surgery in order to achieve the best result, including guidelines on eating, drinking, and taking certain vitamins and medications. Smokers should plan to stop smoking for a minimum of one or two weeks before surgery and during recovery, as smoking decreases circulation and interferes with proper healing.

Recovery Following Tummy Tuck Surgery

For the first few days, your abdomen will probably be swollen and you’re likely to feel some pain and discomfort that can be controlled by medication. Depending on the extent of the surgery, you may be released within a few hours or you may have to remain hospitalized for a day or two.

Your cosmetic surgeon will give you instructions for showering and changing your dressings. Even though you may not be able to stand straight at first, you should start walking as soon as possible.

Surface stitches will be removed in five to seven days, and deeper sutures, with ends that protrude through the skin, will come out in two to three weeks. You will need to wear a support garment over your abdomen and under your clothes for about 6 weeks.

It will take 3 to 6 weeks for you feel like your old self again. Some of our tummy tuck patients return to work after two weeks, while others take three or four weeks to rest and recuperate. Exercise will help you heal better. Even people who have never exercised before should begin an exercise program to reduce swelling, lower the chance of blood clots, and tone muscles. Vigorous exercise, however, should be avoided for 6 weeks.

Your scars may actually appear to worsen during the first three to six months as they heal, but this is normal. Expect it to take nine months to a year before your scars flatten out and lighten in colour. While they’ll never disappear completely, abdominal scars will not show under most clothing, even under bathing suits.

Tummy Tuck Risks & Complications

Thousands of abdominoplasty procedures are performed successfully each year. When performed by a qualified tummy tuck surgeon, such as Dr. Lista, Dr. Ahmad or Dr. Austin, the results are generally quite positive. Nevertheless, there are always risks associated with surgery and specific complications associated with this procedure.

Post-operative complications such as infection and blood clots are rare, but can occur. Infection can be treated with drainage and antibiotics, but may require a hospital stay. You can minimize the risk of blood clots by moving around as soon as possible after the surgery. Our state-of-the-art plastic surgery clinic in Toronto, abdominoplasty patients are given anti-embolic stockings which they are asked to wear for one week following surgery. We also have a specialized device called a compression pump which gently massages the patient’s legs during surgery and, combined with the stockings, greatly decreases the risk of blood clots in the legs.

Poor healing, which results in conspicuous scars, may necessitate a second operation. Smokers are advised to stop, as smoking may increase the risk of complications and delay healing.

You can reduce your risk of complications by closely following your surgeon’s instructions before and after the surgery, especially with regard to when and how you should resume physical activity.
